Reporting an Incident

From time to time, organizations may allegedly be in violation of policy and/or procedure. In these cases individuals may report alleged violations through the following means. Please note that reporting through these means should consider the totality of involvement of and organization and not alleged violations of individuals.

To report violations of the Student Code of Conduct by individual students, please contact the Office of Student Rights & Responsibilities.

°ÄÃÅÁùºÏ²ÊÀúÊ·¼Ç¼ Fraternity/Sorority Incident Report

The Fraternity/Sorority Incident Report form is not submitted anonymously and is intended to be used regarding allegations chapters violating University and/or Governing Council policies, not acts to be considered hazing. This form is reviewed by the Fraternity/Sorority Life Staff before being submitted to either the Office of Student Rights & Responsibilities or the Fraternity & Sorority Life Accountability Board for consideration.

°ÄÃÅÁùºÏ²ÊÀúÊ·¼Ç¼ Hazing Report Form

This form can be filled out anonymously and is intended to be used specifically for hazing allegations. Only with your permission, will your identity will be disclosed to the individuals or group in question. Your identity will be held in confidence. You may submit this form anonymously however; doing so will limit the ability of Fraternity and Sorority Life to fully investigate the incident. You may be asked to submit an official statement on this matter and/or the case may be passed on to student Judicial Programs.

The Greek Anti-Hazing Hotline, established in 2007 by a consortium of national fraternities and sororities, is currently sponsored by 47 national and international Greek organizations.

The Hotline provides an anonymous telephone line for anyone to report a suspected or recent hazing incident to one number 1-888-NOT-HAZE (1-888-668-4293) that accepts calls 24 hours a day. The Hotline connects to a dedicated voice mailbox at Manley Burke, LPA. The calls are automatically saved as audio files that are then transmitted by e-mail to the headquarters of the fraternity or sorority named in the report. This is true whether or not the reports are about one of the sponsors or another organization. In some instances, reports are about athletic teams, bands or clubs. When those calls are received, the institution where the organization is located is contacted.

Accountability Teams and Process

Fraternity/Sorority Life Staff

The Fraternity/Sorority Life (FSL) Staff is responsible for the administration of the FSL Programs Handbook and it's policies. The staff is also responsible for the advisement of the Fraternity and Sorority Life Accountability Board.

Office of Student Rights and Responsibilities

The Office of Student Rights & Responsibilities will review and investigate any and all allegations of Registered Student Organizations, including fraternities and sororities, that is deemed to be beyond the scope of the Fraternity/Sorority Life Accountability Board.

Fraternity and Sorority Life Accountability Board (FSLAB)

To align with the mission of Fraternity and Sorority Life FSL , the Fraternity and Sorority Life Accountability Board (FSLAB) exists to assist in the personal growth and development of our fraternal community by further promoting self governance, accountability, and responsible citizenship. The FSLAB seeks to promote actions that align with the values of °ÄÃÅÁùºÏ²ÊÀúÊ·¼Ç¼
and National/International fraternal organizations and to promote the welfare of the individual members and chapters of the fraternity and sorority community.

The purpose of the FSLAB is to review allegations of misconduct and violations of FSL Policies and the °ÄÃÅÁùºÏ²ÊÀúÊ·¼Ç¼ Student Code of Conduct that involve fraternal social organizations.
The FSLAB seeks to mediate or adjudicate a resolution to said allegations. Under the Code of Student Conduct, the FSLAB has been authorized to review allegations of misconduct involving social fraternities and sororities. Behavioral prescriptions which social fraternities and sororities will be held accountable have been enunciated in the Code of Student Conduct, IFC/MGC/NPHC/PHC constitutions, and FSL community standards.

Fraternity/Sorority Hearing Process

Should an accused organization choose not to accept responsibility for any alleged violation(s) a
Fraternity and Sorority Life Accountabilty Board Hearing Review will be conducted.
